Q: What is care management?

A: Geriatric Care Management is a comprehensive approach to caring for the elderly. The geriatric care manager is trained and experienced in any of several fields related to long-term care, including, but not limited to nursing, gerontology, social work, or psychology, with a specialized focus on issues related to aging. We assist older adults in attaining their maximum functional potential. We are also experienced guides and resources for families of older adults.

Q: What services are provided in Geriatric Care Management?

A: Care Management Services may include:

  • Individual and family consultation and advice
  • In-home assessment
  • Evaluation of health, social and safety needs
  • Development, implementation and monitoring of personalized care plans and associated costs
  • Coordination of medical professionals and community services
  • Advice, planning and arrangement for alternative housing
  • Education and training for clients and caregivers
  • Problem solving, advocacy and crisis management

Q: What is the integrated model we use?

A: At ACS, each client has a Plan of Care developed with his or her care manager. Input is received from the client, family members, loved ones and health care professionals. The care manager ensures that every facet of the client’s life is addressed in the plan. Implementation of the client’s Plan of Care is closely monitored with adjustments made as necessary. Should attendants be needed, they work in concert with the care manager, following the Plan of Care.

Q: How are services paid for?

A: All services are private pay. Medicare and most secondary insurance carriers do not cover care management. Non-medical attendant care is covered by most long-term care insurance policies. We do offer a sliding fee scale for care management in extraordinary circumstances. Please contact the office at (901) 685−3911 for our fee schedule.
